My child will be on time (unless otherwise arranged via note or email) to all after-school and weekend programs hosted by The Weston Collective. A participant can miss no more than 3 days before getting dropped from a program. Students who participate in The Weston Collective’s programs must be 100% committed, willing to be attentive and have a desire to learn the photographic process.
I hereby authorize Zach Weston Executive Director, or another appointed youth advisor, to seek and consent to emergency medical attention for my child as needed; and I further agree to be liable for and to pay all costs incurred in connection with such medical attention.
I hereby release The Weston Collective, its employees, agents and volunteers, from any and all liability, claims, demands, causes of action and possible causes of action whatsoever arising out of or related to any loss, damage or injury that may be sustained by my child while participating in or traveling to and from this event.
